In
consideration of being allowed to enter into the play area and/or
participate in any party and/or program at Inflatable Playspaces LLC,
the undersigned, on his or her behalf, and/or on behalf of the
Participant(s) identified below, acknowledges and agrees to the
following: I represent that I am the parent or legal guardian of any
Participant(s) named below other than myself, or I have obtained
permission from the parent/legal guardian of such Participant(s) to
execute this agreement on their behalf. I agree that the Participant(s)
named below and I shall comply with all stated and customary terms,
posted safety signs, rules, and verbal instructions as conditions for
participation in any party and/or program at the facilities owned or
operated by Inflatable Playspaces, LLC, In addition, if I observe any
hazard during our participation, I will immediately bring it to the
attention of the nearest Inflatable Playspaces employee or official. I
am aware that there are inherent risks associated with participation in
Inflatable Playspaces LLC programs, parties, and/or use of the play
area and inflatable equipment. I either have full knowledge and
understanding of the nature & extent of such risks or I expressly
recognize that there may be some risk unknown to me and sign this
document knowing that it shall be effective with respect to such risk,
I, on behalf of myself and any other Participant(s) named below,
knowingly and freely assume all such risks, including, but not limited
to, personal injury and property damage and those risks that may arise
out of the negligence of other Participants and/or those arising out of
or relating to our participation in any and all Inflatable Playspaces
LLC programs, activities, parties, the use of the play area and/or
inflatable equipment I, for myself and the any other Participant(s)
named below, and our respective heirs, assigns, administrators,
personal representatives, and next of kin, hereby release and hold
harmless Inflatable Playspaces, LLC, its affiliates, officers, members,
agents, employees, other Participants, and sponsoring agencies from and
against any and all claims, injuries, liabilities or damages that I, my
child and/or any Participant named below may sustain, or any damage
that may be caused to property of myself, my child and or any such
Participant in connection with said activities or use of such
facilities or services, including injuries sustained or property damage
caused by any use of equipment. Inflatable Playspaces indoors is an
indoor party facility offering a variety of interactive inflatables.
